Diante das epidemias do SARS-CoV-2 (COVID-19) uma das principais preocupações é a falta de recursos de cuidados intensivos que proporcionem a assistência necessária para tratar os casos críticos, evitando assim um crescente das taxas de letalidade.

Nesse cenário, se um número crescente de pacientes desenvolver doenças graves, devem ser planejados antecipadamente, em nível local e regional, a melhor maneira de gerenciar o aumento potencial na necessidade de recursos de cuidados intensivos; promovendo o acesso a intervenções que salvam vidas, como leitos hospitalares e ventiladores respiratórios suficientes à demanda, de modo que neste momento o compartilhamento de estratégias e soluções é primordial para contermos juntos essa pandemia.














[ OBJETIVO ]

Desenvolver um ventilador mecânico de baixo custo e fácil replicação em impressora 3D ou híbrido com usinagem.

Queremos suprir a necessidade de locais onde não tenham acesso a ventiladores suficientes para casos emergenciais, transporte de pacientes no cenário intra e extra hospitalar, compor em caso de necessidade de mais um leito para ventilação, uso em atendimento pré-hospitalar por unidades especializadas (Serviço de Atendimento de Urgência), salas de exames em hospitais (onde não cabem equipamentos modernos e grandes). Poderá ser utilizado ainda em caso de reanimação cardiorrespiratória de pacientes no ambiente intra e extra hospitalar, visto que sua complexidade de funcionamento e de recursos para funcionamento é mínima.

[ MODELO DE INSPIRAÇÃO ]

Homenagem: Dr. Kentaro Takaoka e o Mini Ventilador 600

História

Em 1951, o Dr. Kentaro Takaoka, médico anestesiologista cansado de tantas dificuldades encontradas na prática profissional, desenvolveu um aparelho de dimensões reduzidas, capaz de executar a ventilação artificial controlada nos pacientes.
Em um laboratório do Hospital da Clínicas de São Paulo, dedicou-se a estudar em um instrumento eficiente, elaborado com peças sucintas e em miniaturas. Testou experimentou seu instrumento em animais por quatro anos.
No ano de 1955 o equipamento, nomeado como Takaoka foi utilizado pela primeira vez no homem, entrando assim para a história. Reconhecido mundialmente, seu respirador tem destaque no âmbito da anestesia até os dias atuais e em meio a pandemia, buscou-se obtê-lo como inspiração para transformar essa tecnologia em 3D.
O equipamento é um ventilador portátil, pneumático e pressométrico e funciona especificamente com componentes mecânicos e não eletrônicos, sendo acionado automaticamente pelo próprio fluxo de oxigênio fornecido ao paciente.
O MINI VENTILADOR 600 ainda serviu de referência para o desenvolvimento de inúmeros outros ventiladores pneumáticos pressométricos. Foi comercializado como Mini Ventilador Takaoka 600 e carinhosamente apelidado de: "Takaokinha ou cebolinha".


Por que deixou de ser usado?

A indústria de ventiladores para respiração artificial foi obrigada a seguir a tendência dos nossos pacientes: cada dia mais complexos em seus problemas de saúde e também nos processos ventilatórios. Para isso, foi necessário criar modelos de ventiladores e ventilação mais complexos e robustos, os quais só puderam vir através de aparelhos microprocessados. Estes possibilitam de maneira rápida, simples e confiável a regulagem do modo ventilatório (pressão ou volume - em suas diversas nuances), tempo inspiratório e expiratório, pausa inspiratória, recrutamento alveolar intermitente e pressão expiratória positiva final (PEEP)..


Por que escolhemos este modelo como inspiração?

Pelos mesmos motivos que o Dr Kentaro Takaoka, em 1951, resolveu produzi-lo. Projeto Brasileiro, extremamente portátil, não necessita de energia elétrica e que feito em impressão 3D, pode chegar a qualquer lugar do Brasil e do Mundo, tendo como tempo necessário para seu funcionamento apenas a impressão, união das peças e uma fonte de oxigênio disponível.

O Breath4Life é uma cópia idêntica do Takaoka M600?
Não, pois hoje usamos modelagem, design 3D e materiais que não existiam na época em que o Dr. Takaoka criou o seu modelo. Para garantir eficiência na vedação de peças impressas em 3D e melhor performance, o desenho das peças foi completamente modernizado e os materiais usados também, como por exemplo imãs de neodímio inexistentes na época do Dr. Kentaro e também nunca usados pela Takaoka na sua fabricação. Além disso, melhorias foram cridas no ramo expiratório para acoplagem de filtros e adicionadas válvulas de segurança para proteção dos pacientes.











[ INDICAÇÕES DE USO ]

- Em pacientes que começam a apresentar necessidade de apoio artificial respiratório invasivo sobre pressão positiva (intubados). Neste cenário, incluímos pacientes sadios (previamente hígidos) que deterioram rapidamente a função pulmonar, necessitando ventilação mecânica e não existam respiradores microprocessados disponíveis (MEDIDA SALVADORA DE VIDAS);

- Pacientes já acometidos por disfunções ventilatórias, necessitando de ventilação mecânica, porém ainda sem um respirador considerado ideal;

- Locais sem recursos financeiros para aquisições imediatas ou de médio prazo de ventiladores microprocessados;

- Locais remotos e de difícil acesso para transporte de pacientes com necessidade de suporte ventilatório ou sustentação de ventilação controlada;

- Transporte de pacientes sob ventilação controlada intra e extra hospitalar;

- Uso por Serviços de Atendimento Médico de Urgência (SAMU), pois ocupam pouco espaço dentro do já exíguo interior das ambulâncias e, normalmente, são utilizados por um curto espaço de tempo (entre a cena do resgate do paciente e o hospital referência);

- Salas e leitos de hospitais convencionais ou temporários sem espaço adequado para um aparelho de tamanho habitual;

- Clínicas de procedimentos de baixa complexidade, onde em sua grande maioria são procedimentos com anestesia local, mas deve possuir um aparelho portátil, simples e barato para possíveis intercorrências (conforme Anexo VI da Resolução do CFM 2.174/2016);

- Dispositivo substituto ao máscara-balão (AMBU) em casos de reanimação cardiorrespiratória: tanto intra quanto extra-hospitalar (atendimento em shoppings, estádios de futebol, grandes reuniões de público);

- Em pacientes na fase inicial de SARA (síndrome da angústia respiratória), antes da evolução com piora do quadro de saúde;

[ LIMITAÇÕES DE USO ]

Este equipamento tem capacidades funcionais e de setup muito exíguos. Por ser uma tecnologia da década de 50, sem a presença de nenhum dispositivo eletro-eletrônico acoplado, apresenta limitações óbvias de regulagem. Portanto não deve ser usado em pacientes que apresentem:

- SARA (síndrome da angústia respiratória) já instalada em sua forma grave;

- Pacientes que necessitem PEEP (pressão expiratória final positiva) como fundamental em sua terapia;

- Locais onde não há equipe técnica treinada par avaliar as conexões e suas integridades, pois o aparelho NÃO POSSUI alarmes e sensores para eventos adversos como desconexão do TOT (tubo orotraqueal);
Observação: Nesses casos pode-se identificar um som metálico característico quando existe a impossibilidade de mandar o ar para dentro dos pulmões (por exemplo: tubo orotraqueal dobrado ou obstruído por secreção pulmonar).





[ CARACTERÍSTICAS TÉCNICAS ]

Fase Inspiratória

Nesta fase, o ventilador opera como um gerador de fluxo constante devido à alta pressão contínua na entrada e também à alta resistência do jato de ar do injetor.

Passagem da fase inspiratória para a fase expiratória

Essa troca é submetida a um ciclo de pressão: ocorre quando a pressão na parte inferior do diafragma, que é a mesma que a pressão na via respiratória, subiu o suficiente, para processar movimento do diafragma contra a força da mola para puxar a válvula de aço que é magnetizável e desmagnetizável (soft-iron) para fora do ímã do anel inferior.

Fase expiratória

Nesta fase o ventilador opera como um gerador de pressão constante, negativo, com resistência substancial em série devido às características do injetor. A pressão gerada e a resistência são altas o suficiente para que o sistema tenha um comportamento próximo de um gerador de fluxo constante expiratório.

Mudança da fase expiratória para a fase inspiratória

Essa troca também é acionada por pressão, ocorre quando a pressão abaixo do diafragma empurra a válvula de aço para longe do ímã do anel superior.
